Mace, 2021: 'No One Should Be Discriminated Against.' Mace, 2026: 'Hospitals for This.'

In 2021, Rep. Nancy Mace called herself 'pro-transgender,' said 'I strongly support LGBTQ rights and equality' and 'no one should be discriminated against,' and co-sponsored the Fairness for All Act. On the September 25, 2026 episode of her Get Maced podcast, she used a former name for her sitting colleague Rep. Sarah McBride, the first openly transgender member of Congress, and told her audience she would 'blanket diagnose' transgender Americans, saying 'they've got mental illness' and 'there used to be hospitals for this.' The medical bodies she invokes moved the other way years ago: the American Psychiatric Association removed the old diagnosis from the DSM in 2013, and the World Health Organization moved gender incongruence out of the mental-disorders chapter in 2019.

Mace, 2021: 'No One Should Be Discriminated Against.' Mace, 2026: 'Hospitals for This.'

"They've got mental illness. There used to be hospitals for this."

Rep. Nancy Mace, on the September 25, 2026 episode of her Get Maced podcast (07:07 to 07:31), on transgender Americans and her colleague Rep. Sarah McBride.

In 2021, Nancy Mace ran as the rare Republican who would say it out loud. She supported LGBTQ rights. She opposed discrimination. She called herself "pro-transgender." Five years later, on her own podcast, she used a former name for Rep. Sarah McBride, a sitting member of Congress and the first openly transgender person elected to it, and said she would "blanket diagnose" an entire class of Americans as mentally ill. Then she reached for the old euphemism for institutionalization: "There used to be hospitals for this." Both sets of words are in the public record. Here they are, side by side.

What Mace said, on the record

At the 07:07 mark of the September 25 episode, Mace turned to Rep. Sarah McBride. She used a former name for the congresswoman, which we do not reprint, and said McBride "cosplays as a woman." From there she widened it to everyone:

"I'm just going to blanket diagnose everyone, every man who thinks he's a woman. Has a mental issue. They've got mental illness. There used to be hospitals for this."

She said it into a microphone, on a show she hosts, as a sitting member of Congress. The clip runs from 07:07 to 07:31.

The "pro-transgender" record she built

Here is the record that sits next to it. When she first ran for Congress, and through her first term, Mace built a brand as a Republican who broke with her party on gay and transgender rights. She said so directly:

"I strongly support LGBTQ rights and equality."

"No one should be discriminated against."

She co-sponsored the Fairness for All Act, which paired federal civil-rights protections for LGBTQ Americans with religious exemptions. She was the lone Republican sponsor of the Serving Our LGBTQ Veterans Act. She described herself as "pro-transgender," framing it as "your body, your choice." Those were her words too.

What the medical record actually says

Mace framed her remarks as diagnosis: mental illness, hospitals. The professional bodies that write the diagnoses moved the other way years ago. The American Psychiatric Association removed "gender identity disorder" from the Diagnostic and Statistical Manual of Mental Disorders in 2013. The World Health Organization's ICD-11, adopted in 2019, moved gender incongruence out of the mental-disorders chapter. The manuals she gestures at do not say what she says they say.

The words are hers, from the public record. In 2021, Nancy Mace said she strongly supported LGBTQ rights and that no one should be discriminated against, and she called herself pro-transgender. In 2026, on her own podcast, she used a former name for a transgender colleague in Congress and said transgender Americans have "mental illness" and that "there used to be hospitals for this." The medical bodies she invokes removed that framing from their manuals more than a decade ago. Readers can watch and judge.
